Self-revoke a Certificate

Use this form to revoke a certificate that has a revocation code. To search for the certificate to revoke, enter an email address or User ID and the revocation code. If you are authenticated by certificate, use the My Certificates checkbox to list all of your certificates that can be self-revoked.
